ECG Gel for Stress Tests & Holter Monitoring

Routine resting ECGs forgive a lot. Stress tests and Holter monitoring do not. The patient is moving, sweating, and the electrodes need to stay attached for either 15 minutes of treadmill or 24–72 hours of ambulatory wear. The gel and electrode choice matters more in these settings than anywhere else in routine cardiology.

Why stress tests stress the gel

During an exercise treadmill stress test, the patient's heart rate climbs, breathing deepens, and within minutes they are sweating. Standard ECG electrodes need to:

  • Stay adhered through sweat and motion.

  • Maintain low impedance as skin hydrates.

  • Resist baseline-wander artifacts from chest movement.

Wet ECG gel on a reusable electrode is less reliable in this scenario than a properly-prepped disposable electrode with solid hydrogel. Most modern cardiology labs use stress-test-rated disposable electrodes — but they still keep wet gel for paddle prep and for back-up.

What makes a "stress-test electrode"

The electrode pad designed for exercise testing has:

  • Aggressive medical-grade adhesive.

  • Conductive solid hydrogel disc.

  • Foam or cloth backing (cloth often performs better with sweat).

  • Pre-printed lead colour coding.

The wet gel still has a role here — for prep of the skin under each electrode if the standard solid-hydrogel pad doesn't grip on sweaty patient skin.

Holter monitoring — 24-72 hours of wear

Holter monitoring is essentially a stress test that lasts days. The challenges:

  • Adhesive failure over 1–3 days of normal activity.

  • Skin maceration under the electrode.

  • Patient comfort during sleep and showering.

  • Lead-wire pull and disconnect.

For Holter, dedicated long-wear electrodes are the standard — not the standard ECG pads. Wet gel is not normally used during the wear; the solid-hydrogel disc is the conductive medium and stays put for the duration.

Skin prep — the critical step everyone skips

Both for stress tests and Holter, skin prep is the difference between a clean trace and an artifact-ridden mess. The protocol:

  1. Shave the electrode site if hairy.

  2. Wipe with 70% IPA to remove skin oils.

  3. Allow to dry.

  4. Light abrasion with a prep pad or fingernail.

  5. Apply electrode.

Most diagnostic-quality issues in stress tests trace back to inadequate skin prep, not to gel quality.

Stock list for a cardiology lab

A cardiology lab running stress tests and Holter monitoring should stock:

Item

Purpose

Wet ECG gel 500 ml

Defibrillation paddle prep; back-up

Stress-test electrode packs

Treadmill stress tests

Long-wear / Holter electrodes

24-72h monitoring

70% IPA wipes / 500 ml bottle

Skin prep

Razors / prep pads

Skin prep

Conductive abrasive paste (optional)

Pre-electrode skin abrasion

Bulk procurement for cardiology

A busy cardiology lab in a tertiary hospital can use 1,000+ disposable electrodes per month and 1–2 L of wet ECG gel. Procurement should be on contract supply with consistent batch quality.

What to ask the supplier

For wet ECG gel: COA per batch — conductivity (μS/cm), pH, viscosity, microbial count. ISO 9001 supplier. Leak-proof bottles. Stable shelf life.

For electrodes: clinical efficacy data on adhesion, on-skin tolerance, hypoallergenic claims if relevant. Sample trial in the lab before contract supply.
