Ultrasound Gel for Physiotherapy & TENS Units

Physiotherapy clinics use ultrasound — but it is therapeutic ultrasound, not diagnostic. Therapeutic units operate at 1 or 3 MHz with energy outputs designed to heat tissue and promote healing, not to make images. The gel still matters; this post explains what physio clinics should buy and stock.


Therapeutic vs diagnostic ultrasound — does the gel differ?

Functionally, no. The same water-based polymer gel that diagnostic centres use is what physiotherapy clinics use on therapeutic ultrasound (TUS) machines. The gel's job is identical: eliminate the air gap between probe and skin, allow the acoustic energy to transmit cleanly.

Two practical differences:

  • Therapeutic ultrasound sessions are longer (5–15 minutes per area) versus seconds of probe contact in diagnostic scans. The gel needs to stay on the surface without drying out.

  • The probe (often called a sound head) is moved continuously; the gel film must remain intact under that movement.

Standard 80,000–150,000 cP viscosity gel handles both well.


What about TENS and electrotherapy?

TENS (transcutaneous electrical nerve stimulation) and EMS (electrical muscle stimulation) use electrical conductivity, not sound. The right consumable for TENS is:

  • Self-adhesive electrode pads with built-in solid hydrogel (most common today).

  • Wet ECG-type gel + reusable electrodes for clinics using older systems.

If a physio clinic is using ECG-style gel for TENS, it is the same product as a hospital uses for ECG — a salt-containing conductive gel, not ultrasound gel. The two cannot substitute.

Format and pack sizes for physio

Most physiotherapy clinics run 5–10 ultrasound sessions a day. Realistic consumption is 2–5 L of ultrasound gel per month. SKUs to stock:

  • 500 ml squeeze bottles at each treatment bed.

  • 5 L jerry can in the supply cupboard for refills.

Higher-volume sports-medicine clinics and rehab centres should size up to multiple 5 L jerry cans or a 20 L drum.


Heat — the warming effect

Therapeutic ultrasound heats tissue. The gel does not heat appreciably during a normal session. Patients sometimes ask whether the gel is "doing" anything therapeutic — it is not. It is the acoustic medium; the heating comes from absorbed ultrasound energy in tissue.

Gel warmers are common in physio clinics for patient comfort. Standard gel works in a warmer; just keep the warmer at 35–40°C and rotate stock.


Skin compatibility for repeat patients

Physio patients return for multiple sessions over weeks. Repeated gel exposure to the same skin patch can rarely cause sensitivity reactions. Best practice:

  • Use paraben-free, fragrance-free formulations.

  • Note any patient reaction in the chart and switch products if needed.

  • Have a small stock of an alternative formulation for sensitive patients.


What clinics should NOT use

  • Petroleum jelly — damages the sound head's matching layer.

  • Massage oils / creams — wrong acoustic match.

  • Hand sanitiser — does not transmit sound; damages probes.

  • Water alone — runs off; no grip.


Sound-head care

Therapeutic ultrasound sound heads are expensive (₹15,000–60,000+ for replacement). Gel residue should be wiped off after every session with a soft cloth; a periodic IPA 70% wipe handles built-up residue. Avoid solvents, abrasive cleaners, and never let the sound head sit submerged in gel.


Choosing a supplier

Same checklist as diagnostic ultrasound: ISO 9001, COA per batch, leak-proof packaging, clear batch / expiry. For physio's longer session times, viscosity stability matters — confirm with the supplier that the gel holds up to 10-minute sessions without drying out.
