The constraint is operator skill (a sonographer or radiologist must be available), and the regulatory layer (PCPNDT registration) is real but manageable. PCPNDT registration: do this first

The Pre-Conception and Pre-Natal Diagnostic Techniques (PCPNDT) Act regulates ultrasound use specifically to prevent sex determination of foetuses. Every centre offering ultrasound — whether or not it does obstetric scans — must register under the Act. The process:

  1. Application to the District Appropriate Authority (commonly the Chief Medical Officer of the district)

  2. Submission of machine details (make, model, serial number), operator qualifications, premises plan

  3. Inspection visit by the Appropriate Authority team

  4. Issuance of registration certificate, valid for 5 years

Operational requirements once registered:

  • Maintain Form F for every obstetric ultrasound performed

  • Display the mandatory PCPNDT signage in the scanning room ("Sex determination is illegal" notice)

  • Sex-determination must not be revealed; no language, gesture or written communication implying it

  • Monthly returns to the Appropriate Authority

  • Records retained for 2 years minimum

Non-compliance carries criminal penalties under the Act. Treat the PCPNDT routine as core to operations, not as paperwork.


Operator qualifications

Ultrasound scanning under PCPNDT may be performed by:

  • Radiologist (MD Radiology, DMRD or equivalent)

  • Gynaecologist for obstetric and gynaecological scans within their scope

  • Sonologist with a recognised qualification (varies by state — verify locally)

Many centres operate with a part-time radiologist (visiting 3–4 sessions a week) plus a senior technician handling the support workflow. The radiologist's signature on each report is mandatory.


Machine selection: three tiers

The ultrasound machine is 60–80% of the capital outlay. Three realistic tiers:

Entry tier (₹4–8 lakh) — refurbished mid-range or entry-new portable

  • 2D B-mode, M-mode, colour Doppler

  • 2–3 transducer probes (convex, linear, sometimes phased array)

  • Sufficient for routine obstetric, abdominal, gynaecological and superficial scans

  • Best fit: tier-2/tier-3 town launches, primary care add-ons

Mid tier (₹10–20 lakh) — current-generation full-feature

  • High-resolution B-mode, full Doppler suite

  • 3–4 probes including high-frequency linear for MSK and small parts

  • Advanced obstetric measurements, 3D/4D fetal imaging (entry-level)

  • Best fit: urban dedicated sonography centres, polyclinic upgrades

High tier (₹25–50 lakh) — premium imaging

  • Premium B-mode and Doppler, contrast-enhanced ultrasound support

  • 5+ probes including endocavity, transthoracic and specialised cardiac

  • Advanced 4D obstetric and cardiac imaging

  • Elastography for breast and liver applications

  • Best fit: tertiary referral, fetal medicine, cardiac specialty

For a first centre, the mid-tier is the safest default. Going low-tier limits the indications the centre can handle; going premium loads capex before referral patterns are established.


Refurbished vs new

Daily consumables: the under-budgeted line

Ultrasound centres run on consumables that look small per scan but add up:

  • Ultrasound transducer gel — the highest-volume consumable; budget 0.5–1 L per 10 routine scans, 1.5–2 L for MSK or obstetric centres doing extended sessions

  • Probe-cleaning wipes or 70% IPA (for low-level probe disinfection between patients) — note: check manufacturer's probe-cleaning compatibility before using IPA on every probe

  • Probe covers (sterile, especially for endocavity)

  • Patient gowns and disposable bedsheets

  • Owner or manager (often the radiologist or family member) — 1
